A changed life

• Note
• This lady has COPD, is extremely anxious has
  panic attacks
• Jan-Oct 2006 24 hospital admissions, 27 days
  IHS
• 57 out of hours calls daily visits from LTC
  team
• Nov - commenced monitoring to now 2 scheduled
  1 day visits to hospital, no emergency calls,
  no out of hours calls, LTC visits reduced to two
  per week
