19Creating the Survey Document
- Enter name of Survey Document here (one document
per Observing System) - Please use naming convention as noted above (ie,
LLL-AAA) - Click on Create button to start

- Survey Type and Creation Date
- Timestamp of last update
- Summary Page for this document lists the pages
in this survey document and the questions on each
page - Click on name of the page to start answering
survey questions
