Why Patient stories

• To find out about patient experiences of their
  local health service.
• To inspire staff and teach future doctors and
  nurses.
• To learn what can be improved and how its been
  done.
• To help spread the Campaigns message and
  achievements.

How do we collect the stories?

• To use a questionnaire to help
• patients to tell us their story in their own
• words.
• When a story collected via a questionnaire is
• particularly relevant to the Campaigns work we
• hope to follow up with a filmed interview.

  experience?
• What would have been the one thing that would
  have made a difference?
• What would be the one message you would tell your
  Local Health Service after your experience?

The filmed interviews

• Only stories with direct involvement with areas
  covered by the 1000 Lives Campaign will be
  considered for filming.
• We will contact the story-teller to ask
  permission to film them telling their stories.
• The patient will be filmed telling their stories
  in their own words.
• They will be filmed in a place convenient to the
  patient by a professional film crew. No one
  involved in the patients experience will be
  involved in the interview.
• They will be used during local, national and
  international events.
• A new consent form will be required.
