What is the Northern Mallee Primary Care
Partnership?

• The Northern Mallee Primary Care Partnership
  NMPCP is a voluntary partnership of local health
  and primary care agencies in Mildura, Ouyen,
  Mallee Track and Robinvale.
• These organisations have come together, with the
  support of the state government to improve the
  planning, availability and coordination of
  primary care and health promotion services.
• The major objective of this initiative is to
  improve health outcomes for consumers of
  services, and their carers.

Service coordination

• Service Coordination involves working with health
  care providers to improve consumer care and
  agency operations such as the ways in which
  consumers are referred to appropriate services.
• One of the many strategies being undertaken to
  address these broader issues is an attempt to
  ensure that relevant and updated information
  about the local services and agencies is
  available.

• The main way we achieve this is through the
  www.connectingcare.com website, an information
  and referral site containing information on more
  than 60 local agencies and over 400 services.
• Health practitioners, agencies and individuals
  can use this website to find information on, or
  refer to the required services.
