Wisconsin Statewide Transition Initiative

Required by IDEA 2004
  • A public agency must provide the child with a
    summary of the childs academic achievement and
    functional performance, which shall include
    recommendations on how to assist the child in
    meeting the childs post secondary goals.
  • Federal Register Vol. 71 No. 156 300.305(e)(3)

For which students?
  • ? Those who will earn a
  • regular high school
  • diploma.
  • ? Those who will
  • age out of high school.

When to do an SoP?
  • Will the student need the information in the SoP
  • to apply to postsecondary education?
  • Complete in early fall and update in spring.

When to do an SoP?
  • Will the student need information included in the
    SOP on employment or independent living
    experiences gained during senior year?
  • Complete in late spring

Purpose of the Summary of Performance
  • Provides documentation that goes
  • with the student when he/she leaves high
  • school.
  • Assists with the documentation for disability
  • for postsecondary education.
  • Assists in the DVR assessment process.

Keep in Mind
  • SoP is individualized and driven by the
  • students measurable postsecondary goal
  • SoP does not require additional assessment
  • SoP is not part of the IEP however, it is
  • best to be completed with the students
  • input (IEP process)

Follow through with the student
  • Give the student and parent a copy
  • Inform the student and parent how and
  • when it can and could be shared
  • Discuss need for a release of info if HS (instead
    of student) shares with postsecondary
    institution.
