7WHAT IS THE ASSEMBLIES OF GOD(AGOM) FUNERALSCHEME?
- It is complete funeral cover for you ( church
member), your family and extended family. - AGOM funeral scheme will help you to lay your
loved ones to rest with dignity and respect.
8WHO CAN JOIN THE SCHEME ?
- Any member of the church aged 84 and below may
join the scheme. - In addition you may cover-
- 1.Your spouse
- Common Law Spouse
- A person recognised by Safrican at its sole
discretion as a Spouse, after a cohabitation of 6
(six) months. - Tenets of Any Asian Religion
- A union of any recognised Asian Culture where
written proof is available. - Tribal Custom
- A person recognised as a legal partner by the
tribal chief and who has written confirmation
that lobola has been paid for such a union. - Full details of the Spouse must be submitted to
Safrican when the Principal Member joins the
Plan. - A divorced Spouse is not covered, unless it can
be proved that he / she is eligible for cover as
a Common Law Spouse or has been nominated as an
Extended Family Member.
Please ensure that the relevant supporting
documents are provided with the application form,
where the details are different from that of the
member .e.g. different surnames. Failure to
provide correct details will in claims being
declined.
9WHO CAN JOIN THE SCHEME ?
- 2. Children
- Unmarried children of the member below the age of
22, this include step , adopted, outside wed
lock and, legal adopted children. Cover may
continue until the age of 25 if they are full
time students at a recognised and registered
educational institution
Please ensure that the relevant supporting
documents are provided with the application form,
where the details are different from that of the
member .e.g. different surnames, children over 21
but still fulltime students
10WHO CAN JOIN THE SCHEME ?
- 3. Extended Family
- Shall mean a parent, parent-in-law, uncle, aunt,
brother, sister, nephew, niece, cousin or
in-Eligible Child of the Principal Member. - Maximum of ten extended family members are
allowed per member

the Department of Home Affairs, e.g.. Your

15What are the rules of the Funeral Scheme?
- Fully Completed Application Forms
- Must be submitted 10 days before the cover
commencement date, - Late submissions will only be processed the
following month - Incomplete Application forms will note be
accepted - Client must keep the outside cover of the
application Form for their records - All dependants must be nominated upfront at the
time of take up the policy
You are not allowed to add additional dependants,
unless its a life changing event such as
marriage, death etc.
16HOW DO I MAKE MONTHLY PAYMENTS EFFECTIVE?
- You have choice from following payment methods-
- At your local church
- From your bank account by debit order
- Salary Stop Order ( For Government Employees)
Premiums are payable monthly in advance, by no
later than 7tht of each month
Please keep records of all your payments
No premiums , no cover, should you miss a premium
the waiting period will start afresh
17WHEN IS COVER EFFECTIVE?
- There is a six month waiting period before cover
is effective for you, your family and extend
family members aged 74 and below - There is a 12 ( twelve) month waiting period
before cover is effective for you, your family
and extend family members aged 75 to 84 - 24 month suicide exclusion ( no cover during the
first 24 months from cover effective date) - Once you have made your first premium payment
you, your family ( Spouse and eligible children)
and extended family members are cover for
accidental death
18HOW DO I CLAIM AND WHAT DOCUMENTS ARE REQUIRED ?
- Claiming your Funeral Cover
- The Claim process is really easy. If you have any
questions please call Safrican on 011 332 0500 or
Your Local Church Representative. - STEP 1 Get a claim form by calling Safrican
on 011 332 0500 between 0845 and 16.45, Mondays
to Fridays , your local church Visit or website
on www.safrican.co.za - and go to the Assemblies of Good Movement
members - STEP 2 Complete and sign the form and
Collect all the necessary documents ( certified
copies of the original)
- CLAIMS PROCEDURE
- SUMMARY
- On the death of any life insured under a AGOM
Funeral Scheme, Safrican must be informed
immediately. A Death Claim Notification Form
together with all the necessary supporting
documents substantiating the claim must be
submitted to Safrican within 6 (six) months from
the date of death. Failure to do so will result
in the benefit being forfeited. - As it is Safricans policy to settle claims
promptly, we request the claimants co-operation
in forwarding the correct and relevant documents,
timeously to prevent any unnecessary delays in
the processing of the claim. - All documents must be clearly certified by the
police or a commissioner of oaths.
19HOW DO I CLAIM AND WHAT DOCUMENTS ARE REQUIRED ?
- AFFIDAVITS ARE NOT ACCEPTABLE
- In addition to the Claim Notification Form and
Proof of Death, the following supporting
documents must be presented - PRINCIPAL MEMBER
- Copy of Identity Document or Passport.
- Copy of the beneficiarys ID and bank statement
reflecting Bank name, account number and account
holders details - SPOUSE
- Copy of deceaseds Identity Document or Passport.
- Copy of Principal Members Identity Document
- Copy of the beneficiarys ID and bank statement
reflecting Bank name, account number and account
holders details - Copy Marriage Certificate.
- Copy of any 2 (two) or more of the following
documents in cases where there is no marriage
certificate or in the case where the surname
differs from that of the member. - Letter from Tribal Chief, signed and stamped.
- Letter providing customary and/or common law
marriage from the Department of Home Affairs (not
an affidavit). - Medical Aid card reflecting dependants details.
- Any legal policy document where the spouse has
been nominated prior to death.
- DOCUMENTATION
- Claims will only be considered upon receipt of
all the necessary documents substantiating the
claim, satisfactory to Safricans requirements. - Documents to be submitted
- CLAIM NOTIFICATION FORM
- A fully completed Claim Notification, providing
the correct policy number and name of the
Principal Member and of the deceased. This must
be stamped and signed by the Policyholder. - Instructions for payment to the beneficiary must
be clearly shown - PROOF OF DEATH
- Original computer produced or faxed certified
copy of the Death Certificate (Bl 5). - or
- Original or faxed certified copy of Abridged
Death Certificate (Bl 20) in respect of
stillborn, together with supporting medical
documents. - Faxed copies of Death Notices must be clearly
certified. Documents certified by a Commissioner
of Oaths, must clearly reflect the name,
signature, organisation, date, address and
telephone number of the Commissioner of Oaths on
the documents.
20HOW DO I CLAIM AND WHAT DOCUMENTS ARE REQUIRED ?
- ELIGIBLE CHILD
- Copy of Principal Members Identity Document
- Copy of the beneficiarys ID and bank statement
reflecting Bank name, account number and account
holders details - In addition, where the surname differs from that
of the Principal Member, 2 (two) of the following
documents will be required - Registration / Birth Certificate reflecting both
parents details (BI-24 / BI-25). - Adoption papers.
- Copy of IRP 2.
- Marriage Certificate Birth Registration in
respect of stepchildren. - Medical Aid card reflecting the Eligible Childs
details. - In addition
- If the child has attained age 22 (twenty two) but
has not yet attained age 26 (twenty six) and is a
FULL-TIME student - Confirmation satisfactory to Safrican, from a
recognised educational institution to confirm
full-time study at the time the death occurred.
Part-time and correspondence students are not
covered.
- If the child has attained age 22 (twenty two) and
is mentally retarded or totally and permanently
disabled, (as determined by Safrican any one of
the following must be submitted - Confirmation satisfactory to Safrican of State
Disability Grant. - Medical Aid application of Principal Member.
- Medical Report.
- STILLBORN
- Proof of existence must be submitted by way of
- Clinical records.
- Medical records.
- Antenatal records.
- Proof of relationship to the member must be
submitted by way of - Certified copies of Identity Documents of both
parents. - Medical Aid card.
- Affidavits from both parents with certified
copies of their ID documentation?? Thought
