With either option, choose 500, 1,000, 1,500
or 2,000 based on income requirements
Both choices offer coverage for the policyowner
for up to 12 months and are guaranteed renewable
to the age of 70. The policyowner must be
disabled within 90 days of a covered accident,
being cared for on a regular basis by a
physician, be employed at least 27.5 hours at the
time of the covered accident, and not be engaged
in any employment or occupation for pay, benefit
or profit. Available via worksite sales and
primary applicant only. Premiums are based on
level of coverage selected.

Sickness Disability rider
500, 1,000, 1,500 or 2,000 per month (All
benefit payments under this rider are subject to
a 14-day elimination period for each sickness.)
  • For a covered sickness, we will pay a monthly
    benefit for up to 12 months when the policyowner

Public Safety rider
  • Law enforcement officers, correction officers or
    firefightersreceive a 2,000 benefit for gunshot
    wounds received while in the line of duty.
    Treatment by a physician in a hospital within
    24-hours.
