- Maximize strategic interface with planning,
oversight and evaluation efforts to achieve
maximal influence on programmatic direction and
content of current state efforts (IPIP and GQI) - Cross train state and regional staff to work with
statewide partners on these efforts (AHEC, CCNC,
NC AAFP, NCPS) - Foster collaboration between local health
departments and primary care practices to promote
patient self management - Pilot new models of care or communication
strategies to improve health outcomes for people
with low health literacy skills.
