Social Services Department

• The responsibilities of the social services
  department at St. Joseph Regional Health Center
  are to assess and evaluate all high risk patients
  that need assistance while in the hospital and
  after they are discharged.

Social Services Department

• Case Managers
• Engage in discharge planning and the transfer of
  patients from the hospital.
• Main service area is clinical needs
• Most Case managers are Registered Nurses.
• In contact with the patients insurance company.

• Social Workers
• Engage in discharge planning and the transfer of
  patients from the hospital.
• Main service area is psychosocial needs (housing,
  drug/alcohol abuse, mental status, financial
  needs, emotional functions etc.)

Application Process

• Contact the Director of Social Work.
• Contact Director of Volunteer Services.
• Complete volunteer packet, attend volunteer
  orientation, and take a TB-test

Rules and Expectations

• Business Casual Dress
• Own Transportation
• No Cologne or Perfume
• Naturally painted fingernails that are cut short

• Lightly Applied Make-up
• Basic Understanding of Office Equipment
  (telephone, fax, copy machine etc.)
• Basic Computer Skills

Duties Responsibilities

• Shadow Social Workers as they review patient
  charts, consult patients, and make referrals.
• Copy and/or fax information for referrals
• Call various facilities and update contact
  information
• Input patient information into computer
• Attend meetings and/or continuing education
  seminars
