CMS Medical Management

• Provides services to resident children from birth
  to age 21 with chronic medical conditions such as
  asthma, diabetes, cleft lip and palate,
  neurology, nephrology and cardiac conditions
• Financial Eligibility Family income up to
  200
• of federal poverty level not Medicaid
  eligible

• Outreach clinics
• Pulmonary - Cleft lip palate
• Endocrine - Genetics
• Neurology - Nephrology
• Available to all medically eligible
• children regardless of income
• or insurance coverage

CMS Family Infant Toddler-FIT

• Service coordination to families with children
  birth to 3 years at risk for developmental delay
• Coordinates Early Childhood Evaluation Services
  ECES
• Advocates assists families in accessing other
  necessary services

CMS Healthier Kids Fund - HKF

• Provides basic primary health care for school age
  resident children with no means to pay
• Eligibility
• - 3 to 19 years old
• - Family income under 300 of federal poverty
  
• guidelines, does not qualify for Medicaid
  or
• any other type of insurance
• Currently frozen to new enrollment

• Dedicated to a system of care that is
  family-centered, community-based,
  culturally-sensitive and coordinated for infants,
  children and families
• Funded by federal Title V Maternal Child Health
  block grant standards for Children with Special
  Health Care Needs and New Mexico general state
  funds
