Recognition and reporting is compounded by
definitions of old age
Chronological or biological age Historically in
health care older adults (geriatrics) referred to
people aged 65 years and older The Commissioner
for Older People in Wales works with and for
people over the age of 60 The NSF for Older
People in Wales refer to people over the age of 50

Age of retirement? Demographic changes- people
are generally living longer The older of the
old that is the 85 years and above is the
greatest growing proportion of the
population Adults may misuse substances at any
age and may begin or continue to or do so in
older age

Life changes may add to problems of recognition
After retirement, there is no employer to note
poor work performance Some older adults
become socially isolated Adult children may live
elsewhere There may be no cohabitant to
recognise the problem Pride- they may hide their
substance misuse Family and friends may
inadvertently enable the clients substance
misuse by denying the problem

Abuse of heroin and other opioids by older adults
is currently relatively rare Substances abused
or misused by older adults mainly include
alcohol, tobacco, prescription and
over-the-counter medications Older people are
vulnerable to substance misuse because they
generally take more prescription drugs than other
age groups Drug interactions are likely to be
worse in older adults because of slowed
metabolism and clearance mechanisms

Misuse and Errors
  • Sensory impairments can also increase the risks
    of error-
  • A person with a hearing problem may not hear
    instructions for taking medications correctly
  • Poor vision may make it difficult to correctly
    read labels
  • Physical ailments such as arthritis, weakness,
and loss of coordination can also pose problems,
such as difficulty opening bottles and strips of
    tablets

Her first experience of trying to buy cannabis
was bemusing and would probably terrify most old
age pensioners, as she explains. "The first
cannabis I bought, I was told to go to a pub in
Newcastle. I stood there looking out of place
with my shopping trolley Eventually Pat found a
dealer who sold her a small bag of cannabis for
20.

Mental Capacity Act
  • The five statutory principles are
  • A person must be assumed to have
  • capacity unless it is established that they
  • lack capacity.
  • A person is not to be treated as unable to make a
  • decision unless all practicable steps to help
    him
  • to do so have been taken without success
  • A person is not to be treated as unable to make a
  • decision merely because he makes an unwise
  • decision

23
Mental Capacity Act
  • An act done, or decision made, under this Act for
or on behalf of a person who lacks capacity must
be done, or made, in his best interests.
  • Before the act is done, or the decision is made,
regard must be had to whether the purpose for
which it is needed can be as effectively achieved
in a way that is less restrictive of the persons
    rights and freedom of action.

24
Test of mental capacity is undertaken in two
stages Stage 1 Does the person have an
impairment of, or a disturbance in the
functioning of, their mind or brain? An example
of impairment or disturbance in the functioning
of the mind or brain may include the symptoms of
drug or alcohol use. Older people may take longer
to recover from these effects
If there is a chance that the person will regain
the capacity to make a particular decision, then
it may be possible to put off the decision until
later if it is not urgent

    particular needs of older people
  • Tailored educational programmes
  • Age appropriate groups/ services
  • Access to services that cater for physical and
    mental frailty

Knowledge, skills and attitudes need to be
influenced across the whole population including
public and professionals
