    Medicaid and the Childrens Health Insurance
    Program (CHIP), LOW-COST or FREE health
    insurance.
  • Why
  • Children who dont feel well, cannot see the
    blackboard, or hear the teacher do not go to
    school ready to learn or to succeed.
  • More than 1 million children in Texas lack health
    insurance Nearly 90 of these
    children have at least one parent that works full
    time.
  • A family of four can earn up to 40,000 a year
    and still qualify! (in 2006)
  • Whats Covered
  • Regular checkups and doctors visits, shots, and
    immunizations, prescriptions, hospitalizations,
    labs, x-rays, and care by mental health and
    medical specialists, vision, hospice benefits.
    CHIP dental benefits have been restored as of
    April 1, 2006.
  • How Do I Apply
  • Its easy! A simple, 3-page mail-in
    application. All thats required is your
    childs social security number and ONE of the
    following proofs of income
  • Copy of one recent pay-check stub (last 60 days)
    OR
  • Copy of your most recent federal income tax form
    OR
  • A letter from your employer, verifying your
    income and should include employee name, the
    average amount earned, length of employment, and
    signed by the employer.
  • (Proofs must be issued for each working parent)
  • Children must be 0 18 years of age and

    U.S. Citizens or Legal Residents of the United
    States.
