Failure Experience Data (FED)
  • Goal Remove non-conforming parts before they
    become part of your product/system and reduce the
    risk of loss of life or property
  • FED document types include
  • ALERTs
  • SAFE ALERTs
  • Problem Advisories
  • Agency Action Notices
  • Lessons Learned

ALERTs SAFE ALERTs
  • Report a problem, i.e., functional failure, or a
    high probability of failure, of
  • Part, component, or sub-assembly
  • Specification
  • Software
  • Facility
  • Manufacturing Process
  • Test Equipment
  • A SAFE ALERT reports a problem that can result in
    loss of life or, property damage

Problem Advisory
  • Same definition as an ALERT
  • Only difference is that a Problem Advisory
    reports a problem that
  • Has an unknown probability or
  • Has a low probability of causing problems for
    other users

Agency Action Notice
  • An Agency Action Notice redistributes problem
    information issued by a government agency other
    than GIDEP
  • An AAN problem can include just about anything
    -- from nuclear reactors to power strips (surge
    protectors)

How Do You Submit FED?

Submitting FED Documents
  • ALERTs, SAFE ALERTs, and Problem Advisories have
    common submittal requirements
  • It must be an objective report
  • Manufacturer must be notified and permitted to
    respond
  • It must bear the GIDEP Reps signature
  • Supporting documentation is necessary
  • FED documents may be submitted by
    non-participants! (Participant Code VV)

  • Instructions for completion are located in
    Chapter 7 of the GIDEP Operations Manual
  • Submitted documents should be
  • Electronic
  • Forms are available on the GIDEP WebPages
  • Mailed hard copy or diskette
  • Forms are available in Chapter 7

  • Titles - Standard Nomenclature, e.g., RESISTOR,
    VARIABLE
  • Lot data codes - extremely important! (YYWW -
    year/year/week/week format)
  • GIDEP adds value by using IHS Haystack database
    to further describe affected part - by adding
    stock numbers and cross reference numbers

  • Submit originals of photos, letters, reports,
    etc. (if possible)
  • Research the problem
  • State ACTIONS TAKEN/PLANNED
  • The more initial effort put in -- the less the
    other 1700 GIDEP Reps have to do!

  • AL/SA/PA require manufacturer notification
  • AL/PA require fifteen (15) working days for
    manufacturer to respond before submitting the
    final copy to the Operations Center
  • SA allows for concurrent notification
  • Notification must be in writing and include a
    copy of the draft document
  • All FED documents can be amended any time after
    committal
