- Follow the directions and read all instructions
- Each application may require something different
- Applications are accepted by the colleges in
separate pieces at separate times - (check college for specific instructions)
- The Counseling Center requires 15 school days to
process our section of the application - 3 weeks excluding weekends

- The application consists of several sections

- Complete Part I of the application
- includes the student's personal information along
with extra-curricular, honors and awards,
community service and required essays - Submit student portion of application on-line
with required fee - Contact the College Board or ACT Testing Company
to send official score reports - continued

- Speak with teachers regarding writing letters of
recommendation - Click on Icon for College Application Procedure
directions - Provide teachers with one envelope per college
addressed to the school with one stamp - Complete a Teacher Recommendation Questionnaire
and Recommendation Request Form for each teacher
(available in the guidance office) Click on Icon
for Questionnaire Form - Give teacher evaluation form from each college to
selected teachers - Bring Counselor the forms each college is
requesting - Complete Transcript Request Form for each college
(available in the guidance office) - Submit Large Manila Envelope addressed to each
college with 4 stamps - Remember the 15 school day deadline

- Complete the Secondary School Report Form or
Guidance Counselor form. - The Counseling Center will mail the completed
forms along with the following - Official Transcript
- List of Senior Year Courses
- Letter of Recommendation by counselor
- Smithtown High School Profile
12Counseling Center Application DeadlinesWith over
400 seniors in each high school, The Counseling
Center requires 15 school days to process its
portion of the application. Below are common
college deadline dates and the date the
counseling section must be submitted in order to
guarantee meeting those college deadline.
- College Deadline Submitted to Counseling Center
by - November 1st October 7th
- November 15th October 21st
- December 1st November 4th
- December 15th November 22nd
- January 1st December 5th
13Teachers Section and Responsibilities
- Teacher will complete teacher evaluation forms
the college is requesting - Write a letter of recommendation for the student
- (Remember to allow adequate time for teacher to
write a meaningful letter. Some teachers get
asked to write letters by many students) - Teacher will either mail the teacher evaluation
form and the letter of recommendation in the
stamped, addressed envelope the student provides
or submit this information on-line
14Colleges Responsibilities
- Create a file for the student upon receiving any
piece of the application - Collect all pieces of the application
- If anything is missing, the college may contact
you. It is best to contact the college directly
to make sure your entire application was
received. (including student, guidance counselor,
teacher and SAT and/or ACT information). Some
schools have online application status updates.
Remember there is often an overlap time with
mail. - Once everything is received, the file will be
reviewed - The college will make you aware of their decision
pending their timeline
15Where You Should Be Now
- Have a list of colleges you are applying to that
includes safety, target, and reach schools. - Secure two teachers to write letters of
recommendation. - Complete Student Profile and turn it in to
Counseling Center. - Draft essay and have it reviewed by teacher.
- Start filling out your part of the online
application. - Make up envelopes for The Counseling Center
teachers to submit.
