Facial Hair Policy
The shaded areas are the respirator seal
areas. Facial hair is not permitted on parts of
the face

PPE TRAINING

• Tool Pusher and all crew members had some type of
  facial hair.
• No H2S Monitors
• A few had H2S Training but had never been FIT
  tested or worked with an SCBA.
• Had to look at training cards to tell me if they
  had H2S Training.
• Area that we were going to be drilling is known
  for high concentrations of H2S.

Actions Taken
STOP THE JOB

• Called Manager and briefed him on what we were
  going to do.
• Called a Safety Company and had them send someone
  trained to be on site 24 hours a day until we
  could get the crew trained and fit tested.
• Called Drilling Company Manager and Drilling
  Company Safety Manager and offered to take care
  of getting their crews trained.
• Had crews fill out Medical Evaluations, once
  approved, had H2S trainer come to site. Certify
  and Fit test crews,
• Total Time 5 Days
