DRY EYE
  • Dry eye is a disease of the ocular surface
attributable to different disturbances of the
natural function and protective mechanisms of the
external eye, leading to an unstable tear film
    during the open eye state.

Surv Ophthalmol 2001 45(2), S199-202
10
Dry eye
  • Also known as
  • Keratoconjunctivitis sicca (KCS)
  • Dry Eye Disease (DED)
  • Dry Eye Syndrome (DES)
  • Ocular Surface Disease (OSD)

11
Symptoms
  • Irritation
  • Redness
  • Burning/ Stinging
  • Itchy eyes
  • Sandy- gritty feeling (foreign body sensation)
  • Blurred vision
  • Tearing
  • Contact lens intolerance.
  • Increased frequency of blinking
  • Mucous discharge.
  • Photophobia (less frequent symptom)
  • Symptoms worsen in windy or air-conditioned
    environments.
  • As day progresses.
  • After prolonged reading, working on computers

Supl PT Digest 2003, 28(12), 1-45
12
  • Dry eye can be a chronic, debilitating condition.
  • Persistent, uncontrolled dry eye can lead to
corneal erosions, corneal defects and in severe
    cases corneal ulcers.

13
Dry eye may be sight-threatening

STERILE PRESERVATIVE FREE OCULAR
LUBRICATING OINTMENT
  • White Soft Paraffin ( 57.3) and Liquid
    Paraffin (42.5)

PHARMACOLOGY
  • When white soft paraffin is applied to the
lower eyelid, it melts at skin temperature and
gradually moves over the skin on to the ocular
surface in a time released fashion. This movement
    of petrolatum is termed supracutaneous.
  • Forms an occlusive film over the surface of
the eyeball and improve the composition of the
tear film by enhancing the mucin and aqueous
    layers.
  •

20
PHARMACOLOGY (Contd.)
  • Slows evaporation of the tear film.
  • Thus, it breaks the vicious cycle in which dry
eye leads to blepharitis which leads to dry eye.
  • Prevents frictional damage to the ocular surfaces
secondary to lid movement or extra ocular
    movements.
  • Acts by retaining fluid and maintaining hydration
    of the ocular surface.

Eye, 2006 pg183
21
PHARMACOKINETICS
  • Onset of action begins within 10 minutes of
application, peak at 30 minutes, while
concentrations at 6 hours were still at levels
    near 20 of peak concentration.
  • White soft paraffin remains in the eye for a
    longer time.

22
AD-LUBE EFFICACY
  • There are studies and case histories evaluating
efficacy of white soft paraffin . These conclude
    that

23
ADLUBE EFFICACY HIGHLIGHTS
  • Effective in reducing evaporation rate and
blinking rates in patients with dry eye.
  • Prevents dessication from corneal tear film.
  • Statistical significant improvement observed in
subjective symptoms of vital staining of ocular
    surface.

British Journal of Ophthalmology, 1999 83
767-770
24
ADLUBE EFFICACY HIGHLIGHTS
  • Reduces burning sensation in patients with dry
    eye and blepharitis.
  • Reduces reflex tearing in patients with dry eye
    and blepharitis.
  • Reduces the need for artificial tears and warm
    compressors.

25
ADLUBE EFFICACY CONCLUSION
  • Well tolerated by the patients, and provides a
totally new therapeutic approach providing steady
levels of medication to the ocular surface which
offers additional benefits in the management of
    dry eye.

ROLE OF WHITE SOFT PARAFFIN IN CRITICALLY ILL
PATIENTS
  • Microbial keratitis has been reported among
critically ill patients and the need for
effective eye care in the intensive care unit
(ICU) has been recognised for some time.
  • A recent survey in UK found that 25 of ICUs
used ocular lubricants as eye care.

British Journal of Ophthalmology,
2005891068-1069
27
ROLE OF WHITE SOFT PARAFFIN IN CRITICALLY ILL
PATIENTS
  • A condition where the Patient does not close its
eyes completely when sleeping, causing the drying
of the cornea, Inflammation and irritation of the
    cornea.
  • Studies documenting in ICU patients who developed
exposure keratitis using white soft paraffin
    proved that it is effective.

RESULTS OF PRESERVATIVE EFFICACY TEST
  • CONCLUSION
  • The sample is preserved efficiently.
  • REULTS
  • As evident from preservative efficacy data,
ADLUBE is self preserved and does not require any
    additional preservative.

30
INDICATIONS
  • For the temporary relief of burning, irritation
and discomfort due to dryness of the eye or due
to exposure to wind or sun.
  • Also may be used as a protectant against further
    irritation.

31
OTHER INDICATIONS
  • Exposure keratitis
  • Decreased corneal sensitivity
  • Recurrent corneal erosions
  • Blepharitis
  • Lids and conjunctival problems
  • Tearfilm problems

Dosage and administration
  • For topical Ocular administration.
  • Wash the hands before use.
  • Pull down the lower lid of the affected eye and
apply a small amount (one fourth inch) of
ointment to the inside of the eyelid. (Ideal for
    use at bedtime).
  • There is no variation in dosage for age.
  • To avoid contamination, do not touch tip of the
container to any surface. Replace cap after
    using.

33
CONTRAINDICATIONS
  • Contraindicated in patients hypersensitive to any
of the ingredient in the product or lanolin
    alcohols.
  • Contraindicated for use in bacterial, viral or
fungal infections in the eye, unless used along
    with an anti infective medicine.

34
PRECAUTIONS AND WARNINGS
  • To avoid contamination during use, do not touch
the tube tip to any surface.
  • If irritation, pain, redness and changes in
vision occur or worsen, treatment discontinuation
should be considered and a re-evaluation of the
    patient's condition should be made.

http//www.emc.medicine.org.uk
35
PRECAUTIONS AND WARNINGS
  • Contact lenses should not be worn during
instillation of the drug. After instillation
there should be an interval of at least 30
    minutes before reinsertion.
  • Concomitant topical ocular medication if
necessary, there should be an interval of at
least 5 minutes between the two medications.
AD-LUBE should always be the last medication
    instilled.

http//www.emc.medicine.org.uk
36
DRUG INTERACTIONS
  • No interactions have been observed with
ADLUBE, since the constituents have a well
established medicinal use, no interactions are
    anticipated.

ADVERSE EFFECTS
  • ADLUBE is safe for ophthalmic use.
  • It is non-toxic and proved for food and medicinal
    use.
  • It causes temporary blurring of eye vision which
    may last for 1-15 minutes.
  • Irritation, stinging and burning sensations have
    been observed rarely (1/10,000,
  • Allergic reactions, pain hyperemia and
conjunctivitis have been reported very rarely
    (
