What is a Logic Model?

• A logic model is a graphic representation of a
  programs theory of change
• It describes and sequentially links specific
  conditions with defined program activities to
  achieve intended outcomes

Core Values

• Most basic beliefs that are the foundation of the
  PAT model program
• Flow from the programs vision and mission
• Define PATs target population as young children

• Key principles of high quality programming as

• Provide the rationale for the PAT model components

• Summarize the PAT services that directly support
  children and families
• Flow directly from a set of assumptions

• Specific statements of program services provided
  as part of the model
• Guides implementation
• Fully implementing activities will lead to
  desired outcomes

Short-term Outcomes

• Are anticipated consequences that result directly
  from program services
• May be changes in attitudes, behaviors,
  knowledge, and skills on an individual level
• Help focus resources and staff effort on
  specific, realistic goals

Intermediate Outcomes

• Due to program implementation indirectly or
  building upon short-term outcomes
• Are measurable at a later time point

• Require greater time to measure
• May be difficult or expensive to measure
