4He has a rare, near fatal, condition. If he
could talk, he could tell you about it.
Fortunately, Dr. Jones figured it all out just
before he left on his missionary trip to
Africa. Its all in his records at Sacred
Heart Hospital!

System at Sacred Heart Hospital! Heart is our

user. If you believe you should have access
to these records, contact our security office
between 9 AM and 5 PM Monday through Friday.
7 Understand that unless your patient has
completed a release of information form which
lists you specifically, your request for
access will be reviewed by our security
authorization committee. These reviews
normally take 3-5 working days.
8 If you do not have privileges at this
facility, you will need to hand deliver your
original medical diploma and your original
license in this state. Credential review,
including verification of your credentials in
every state where you have ever practiced,
takes approximately 30 days.
9 We are quite sure you understand our concern

10Its too bad we cant use Dr. Jones work up. I
guess well have to start over. We should be
able to start treating him in 2-3 days. Here is
a list of tests we need to do today.
11Oh, by the way, based on the evaluation of his
credit and insurance status, he should be able to
pay for this hospitalization. His Platinum
Master Card showed a 20,000 limit within 2
minutes of his arrival at the ER.
