GUIDELINES IN THE ISSUANCE OF NEW CERTIFICATE OF
REGISTRATION UNDER THE PHILIPPINE COOPERATIVE
CODE OF 2008 (RA9520)

Coverage
  • These Guidelines shall cover the issuance of new
    Certificate of Registration of all types and
    categories of registered and confirmed
    cooperatives that are existing prior to 22 March
    2009 except for cancelled and dissolved
    cooperatives.

Period to Register The Registration shall start
immediately upon the effectivity of the
Guidelines and ends on March 22, 2010.

Requirements
  • original copy of certificate of registration or
    certificate of confirmation including certificate
    of amendment/s.
  • photocopy of registered articles of cooperation
    and by-laws including registered amendment/s
  • copy of latest audited financial statements as
    o of December 31, 2007 and/or 2008
  • and or 2009

Fees
  • The cooperative shall pay the basic registration
    fees in accordance with MC 2004-07 on the
    registration of cooperatives depending on their
    category, to wit
  • Primary Cooperatives --- Php 500.00
  • Secondary Cooperatives --- Php 2,000.00
  • Tertiary Cooperatives --- Php 3,000.00

Venue for Registration
  • Primary or secondary cooperative -nearest CDA
    Extension Office (EO) having jurisdiction over
    them.
  • Tertiary Cooperative Federations and Unions and
    other types of cooperatives - CDA Central Office.

Procedures
  • cooperatives with on-line access
  • CDA Website, www.cda.gov.ph
  • Cooperative Identification Number (CIN)
  • Submit the required documents together with the
    checklist within 15 days from the date of notice

Procedures
  • 2. cooperatives without on-line access
  • proceed to the concerned CDA Extension or Field
    Office for assistance
  • union or federation of cooperatives
  • Cooperative Development Office of LGU
  • NERBAC in the regions

Non-Operating Cooperatives
  • Cooperatives classified as non-operating are
    advised to proceed immediately to the concerned
    CDA Extension Office for evaluation of their
    status.
  • Dissolved and cancelled cooperatives shall not be
    eligible for the issuance of new Certificate
    of Registration under RA 9520.

Issuance of new Certificate of Registration
  • A new Certificate of Registration shall be
    issued to all qualified cooperatives in the form
    and style as approved and replaces all previous
    certificate of registration or certificate of
    confirmation.
  • A cooperative shall have one (1) year to amend
    their article of cooperation and by- laws to
    conform with the provisions of RA 9520.

Issuance of new Certificate of Registration
  • Three (3) copies of the new Certificate of
    Registration (CoR) shall be printed and shall be
    distributed, as follows
  • Original - Cooperative
  • Duplicate - CDA Extension Office
  • Triplicate - CDA Central Office
  •

Issuance of new Certificate of Registration
  • The old / surrendered Certificate shall be
    stamped REPLACED upon the issuance of the new
    Certificate of Registration. Senior CDS shall
    affix signature on the surrendered
    certificate.

Registration Assistance Center (RAC)
  • All EOs and Field Offices shall establish (RAC)
    to provide assistance by
  •  accomplishing the application form
  • receiving documents from applicants

Report Generation
  • Management Information System (MIS) Staff shall
    generate monthly statistical report and master
    list of all registered cooperatives.
  • The master list shall contain the cooperative
    name, address, registration number, and date of
    registration.

Transitory Provisions
  • Cooperatives shall continue to legally exist for
    the period provided in their original Articles of
    Cooperation, unless previously amended.
  • Cooperatives which failed to comply with Art. 144
    RA 9520 up to March 22, 2010 shall be deemed
    cancelled motu proprio.
  • CDA shall cause the cancellation of such
    cooperatives.
