3Real Choice Systems Change Grants for Community
Living
- Offered by Congress since 2001 as a mechanism to
assist States with building infrastructure that
will result in effective and enduring
improvements in long-term support systems. - Long-term support systems enable people with
disabilities or long term illness to - Live in the most integrated community setting
appropriate to their individual support
requirements and preferences - Exercise meaningful choices about their living
environment, the providers of services they
receive, the types of supports they use, and the
manner by which they are provided and - Obtain quality services in a manner as consistent
as possible with their community living
preferences and priorities.

- Since 2001, the Centers for Medicare and Medicaid
Services (CMS) has awarded over 297 grants to all
50 states, DC, and two territories totaling 238
million - Virginia has received six grants since 2001 to
support Systems Change - Real Choice Systems Change (2001) - DMAS
- C-PASS (2002) Partnership for People with
Disabilities - Rebalancing Initiative (2004) Partnership for
People with Disabilities - Transforming Virginias Mental Health Services
System (2004) - DMHMRSAS - Aging and Disability Resource Center (2005) VDA
- Family to Family Health Care Information and
Education Center (2005)

- CMS intends to award grants for Fiscal Year 2006
in the following grant programs - Aging and Disability Resource Center (ADRC)
Grants - Systems Transformation Grants
- Virginia is not eligible to apply for the ADRC
Grant because we received the ADRC Grant last
year

- Intent Provide States with a greater level of
support to begin or further current initiatives
that target key elements of systems
infrastructure - Funding States may request up to 3.0 million
to support development and implementation of a
Systems Transformation Grant for a five (5)-year
period - Grant Goals States are required to address at
least three (3) of the six (6) transformation
goals CMS has designated as critical to systems
transformation

9Systems Transformation Grant Grant Phases
- Grant Phases
- Application Need to conduct a Systems
Readiness Assessment that describes the
applicants progress toward the goals of the
systems transformation - Planning Phase States use the first 9 months of
the grant to develop a vision and a strategic
plan for the components of their long-term
support system need to present to CMS - Implementation Phase Once strategic plan is
approved by CMS, grantees will have the remainder
of the grant to implement grant goals
10Grant Technical Assistance and Evaluation
- CMS will provide funding to grantees to obtain
technical assistance from a contractor of their
choice to support the grantee with critical tasks - Development of a strategic planning template
- Measurement of outcome indicators
- Consultation as needed to accomplish goals
- Total cost of TA contract cannot exceed 20 of
proposed budget - Applicants are also required to develop and
implement a systems and impact-based evaluation - Funding for the evaluation cannot exceed 15 of
the total grant award -

- HHS plans to award this grant in 2006 to 6-7
states. - Letter of intent was due May 11, 2006
- Application is due June 15, 2006
- Awards will be given to grantees by September 30,
2006 - The Office of Community Integration, DMAS,
DMHMRSAS and other key stakeholders are
collaboratively developing and applying for this
grant

17Stakeholder Involvement
- CMS requires states to demonstrate a meaningful
inclusion of key stakeholders, including
consumers and advocacy groups, in both the
Planning and Implementation phases of the grant - The Office of Community Integration, DMAS,
DMHMRSAS, and other key stakeholders, with the
support of Secretary Tavenner, are actively
soliciting the involvement of stakeholders - To develop consensus on the goals and objectives
selected for the grant - To assist in the development and review of the
grant application - To demonstrate their support for grant activities
by submitting letters of support and commitment
to action if the grant is received
