    local, state, and county government.

8
Partnership with Physical Health Care
  • In January, 2008, Apalachee partnered with Leon
County Health Department to provide psychiatric
coverage and benefits restoration in Bond
Community Health Clinic (a federally qualified
health center) and Neighborhood Health Clinic.

9
Development of a Partnership
  • Apalachee placed a psychiatric ARNP on-site at
the two community health centers for 32 hours per
week, and a case manager for 40 hours per week.
  • Leon County Health Department funded these
    positions at 142,808.00

10
Services provided
  • Between January 2008 and November 2008, 420
clients were seen through this program. Leon
County population is about 273,000, and the
Apalachee Center Leon County outpatient clinic
serves about 900 unduplicated adult clients
    annually.
  • During 07/08, occupancy on the Apalachee Crisis
Stabilization Unit fell below 80 for the first
    time in five years.

11
New Directions
  • In November, 2008, Bond Community Health Clinic
received a grant award for the implementation of
the IMPACT model. They contracted with Apalachee
Center for expanded time, including 32 hours a
week of psychiatric ARNP/MD time, and 40 hours
per week for a Masters level substance abuse
    counselor, for approximately 150,000.

12
New Directions
  • Currently, Apalachee Center and Bond CHC are
planning to bring primary care services on-site
at Apalachee Center 12 hours per week, in order
to provide desperately needed medical services
for the severely and persistently mentally ill
population. This will establish Apalachee Center
as a satellite site for Bonds FQHC services.
