Benefits Eligibility and Enrollment Information

• Benefits Eligibility Employees are eligible for
  benefits if appointed half-time or more for a
  period of more than six (6) months. All Tenure
  Track employees and Lecturers/Coaches appointed
  for two or more consecutive quarters on one
  appointment letter at .4 time-base (6 WTUs) or
  greater are also eligible for benefits.
• Effective Dates of Coverage If you enroll on or
  before the last day of the month in which you are
  appointed, the effective date of medical and
  dental coverage will be the first day of the
  following month. The effective date for FlexCash
  in lieu of medical and/or dental, vision and all
  other benefits is the first day of the second
  month (Example If you enroll by January 31,
  2006, medical and dental will be effective
  February 1, 2006, FlexCash in lieu of medical
  and/or dental, vision and all other benefits will
  be effective March 1, 2006).
• Information/Material to Bring Copy of your
  appointment letter If married, your spouses
  Social Security Number, date of birth, and
  photocopy of your marriage certificate that must
  be provided to the Benefits Office within 30 days
  of your hire date. If enrolling a Domestic
  Partner, contact Benefits Office for required
  documentation.
• Enrollment Period You have 60 calendar days
  from your appointment date to enroll in benefits.
