Required by IDEA 2004

• A public agency must provide the child with a
  summary of the childs academic achievement and
  functional performance, which shall include
  recommendations on how to assist the child in
  meeting the childs post secondary goals.
• Federal Register Vol. 71 No. 156 300.305(e)(3)

• 44 of SoPs completed included Reading/Math
  information
• 65 included a statement of overall functional
  performance
• 70 included recommendations to help students
  achieve their postsecondary goals

Disability Documentation

• Based on discussions with the postsecondary
  institution, compile and attach documentation
  that demonstrates that a disability currently
  exists.

Follow through with the student

• Give the student and parent a copy
• Inform the student and parent how, when
• and why it can and could be shared
• Discuss need for a release of info if HS (instead
  of student) shares with postsecondary
  institution.
