• All cases that are eligible are registered to
  0525 PRIOR to tissue submission per section
  5.1
• Patients begin concurrent treatment

• Tissue is submitted to Dr. Aldape _at_ MDA with the
  completed P4 form, ST form and PI (pathology
  report) per section 10.0
• 1 cubic centimeter of tissue is required

• Dr Aldape will send an e-mail with the completed
  P4 form indicating histology eligibility
• If the case is ineligible-pt continues w
  concurrent treatment-TMZ will be supplied

• If there is not enough tissue for the MGMT
  analysis you may be asked for a 2nd tissue
  submission
• When MGMT analysis is completed and results are
  received at HQ and e-mail will be sent informing
  you that the 2nd step registration can be
  completed

• All data collection forms are to be submitted via
  the web. Paper forms will be returned
• Revisions to CRFs must be made on the original
  WEB submission (complete summary) print out
