their unique abilities. In the content of
community access and participation, fidelity
support  services involves providing individuals
with the necessary resources, support, and
guidance to fully participate in and engage with
their community. This includes things like
planning appointments, providing transportation,
assisting with communication, providing
accommodations for physical or cognitive
disabilities, and helping individuals to build
social connections and relationships within the
community.

24/7 In-home Independent Living Support
We establish fidelity in residential support for
our customers by creating a consistent and
reliable system of care that meets the
individual's specific needs and preferences. This
involves developing or reviewing a support plan
that outlines the tasks and responsibilities of
staff, as well as setting clear expectations and
boundaries for the individual receiving
support.Here are a few steps that we take to
establish fidelity in residential support for

needs and preferences It's important to involve
the individual in the care planning process as
much as possible. This can help to ensure that

Nursing Services
From monitoring your health conditions to
administering your medicines, we offer all types
of nursing aids you need. Our nurses work closely
with other members of the support team, including
doctors, therapists, and support workers, as well
as with the individual and their family, to
ensure that the support provided is coordinated
and comprehensive. Fidelity Support Services have
goal-oriented nurses who assist individuals to
overcome some of the challenges in achieving
their goals and reach their full potential, by
providing the necessary nursing care and support.
This may involve working with the individual to
develop a plan of care that is tailored to their
specific needs, preferences, and goals, and
connecting them with the appropriate resources
and services

Support Coordination and Specialist Support
Coordination
Participants with the need for targeted goals can
have a programme designed specifically for
them. Our support coordinators and specialist
support coordinators work closely with
individuals and their families to identify their
needs, develop a plan of support, and connect
them with the appropriate services and
resources. We also provide ongoing support and
follow-ups to ensure that the supports and
services provided to the individual are
consistent and reliable over time. Programmes
include but are not limited to individuals
accessing community resources and services,
recreational programs, transportation services,
employment opportunities among others.
