Problem

• Lack of effective communication with physicians
  leads to increased readmission.
• Communicate resident needs and the type of care
  that can be delivered.
• Establish that communication failures lead to
  resident harm.

Intervention

• SBAR - establish communication pathway with
  physician
• Can be adapted to any clinical situation
• Establish target group for implementation Lower
  Respiratory Infections

• Education Educate nursing supervisors in
  regards to tool use.
• Physician Notification Letter forwarded to
  physicians regarding implementation.
• Reinforce compliance most common hurdle was
  time constraints. Takes too much time to fill
  out.
• Reinforce benefits of SBAR teamwork, safety,
  expectations, critical thinking skills

• Complete monthly audits of readmissions to
  enhance compliance.
