    and development. The team utilizes a system
    approach that is student and family centered
    solution focused and culturally competent.

School Based Team How?
  • Principal designates SBT Leader and other Team
    members
  • SBT Leader, with administration, sets time, day
    and location of weekly meetings
  • SBT Leader identifies the academic and school
    performance data needed before the initial
    meeting, i.e. SSAASY, EDW, school records
  • SBT Leader creates an agenda

School Based Team How?
  • Members sign the Attendance Log Confidentiality
    form
  • Team reviews Referral form and all student data
    from teachers, parents, student records, SSAASY,
    etc.
  • Team identifies available supports and
    interventions targeted to the specific concern
  • Team initiates a Student Action Plan
  • Team identifies the goal for the student and the
    strategies designed to meet the goal

School Based Team How?
  • Team members are identified as Case Liaison for
    individual students
  • Case Liaison notifies parent (guardian) and
    teachers of the components of the Student Action
    Plan
  • Team maintains ongoing progress notes on all
    cases
  • Team establishes the timeline for implementation
    and evaluation of the Student Action Plan

  • Team documents outcomes using the Tracking Report
    form, which is submitted monthly
  • At the end of the year, SBT Leader will submit
    the Summary Statistical Report
  • At the end of the year, SBT Leader will receive a
    stipend

Community Agencies
33 agencies have cooperative agreements to
provide behavioral health services
  • Agency staff must sign the SBT attendance log and
    confidentiality statement
  • Agency staff must complete a background check
  • Agency staff must be covered by insurance
  • Agency staff must maintain confidentiality about
    student/school issues
  • Agency staff may be invited to attend the meeting
    by SBT, Department of Juvenile Justice (DJJ) or
    Department of Children and Families (DCF)

BAKER ACT DECISION TREE
The Baker Act is used to provide effective
care to a student whose behavior is a danger to
self and/or others.

Situation not de-escalated CONTACT PARENT(S)
/legal guardian and obtain releases before
student leaves
Student returns to class
Staff meets with parent and student referred to
the School Based Team
