• There are currently 7000 patients with diabetes
  in Western Sussex PCT
• By the year 2010, it is estimated that this is
  likely to double
• The need to increase awareness and education for
  patients and health care professionals

• Local/Accessible services closer to the patients

• Improve awareness and education

• Accessible to specialist services
• when appropriate

  in the community
• Support, education and clinics in GP Practices
• Group patient/carer education sessions
• Local enhanced service for GP Practices
• Reducing demand on specialist secondary care

When will this happen?

• Staff consultation underway
• Implementation groups are formed
• Education programme for health care professionals
  has commenced
• Local enhanced service in some practices now
• Diabetes specialist team will visit practices on
  a regular basis from January 2006
