State the Aim Clearly
  • Teams make better progress when they are specific
    about aims
  • Aim statement should describe the system and
    patient/resident population to be improved
  • Ensure that aim gives guidance on approaches
    improvement

Include Numerical Goals
  • Include numerical goals that require fundamental
    change to the system
  • Help create tension for change, directs
    measurement, focuses on initial changes, and
    determine level of support needed
  • Example Reduce pressure ulcers vs. Reduce
    the prevalence of pressure ulcers by 24 within
    12 months

Set Stretch Goals
  • Communicates immediately and clearly that status
    quo is not an option
  • Goal cannot be met by tweaking system
  • If stretch goals are clear, people will look for
    ways to overcome barriers to achieve goal

Avoid Aim Drift
  • Clearly Indentify Timeframe
  • Start each meeting with explicit statement of aim
  • Review progress quantitatively over time

  • Every team needs to recognize when to refocus
  • May need to narrow focus on smaller part of the
    system
  • Not to be confused with Aim Drift
