2Claim Filing Process for Injuries Occurring
Outside of the US and US Territories
- Initiating claims filed on CA-1s for traumatic
injuries and CA-2s for occupational diseases
where the injury or exposure location is outside
the United States or U.S. Territories, must be - sent to the Cleveland District OWCP Office
- coded with 09 in the OWCP District field of
the EDI claim form for initial adjudication.
3- If you are submitting a paper claim for injuries
that occur overseas you should fax the completed
claim form to the Cleveland District OWCP Office. - The Fax numbers for the Special Claims Unit are

Woods, Special Claims Unit.
4- The Department of Labor has created a special
numbering scheme for claims associated with the
war and reconstruction activities in IRAQ. - Be sure to identify cases as IRAQ on CA-1 or
CA-2 - The numbering associated with the war in IRAQ is
09-3002XXX.
5- If you are mailing a paper claim form it should
be sent to the Cleveland District Office,
Attention of David Woods, Department of Labor,
Office of Workers Compensation, 1240 E. 9th
Street, Room 851, Cleveland, Ohio 44199. - The envelope should be marked DO NOT OPEN IN
MAILROOM. Put this on in large bold letters
6Following initial adjudication, cases will be
transferred to the district office that has
jurisdiction over the state where the claimant
resides. When the claimant resides outside the
U.S. or U.S. Territories, the case will remain in
the Cleveland office. This process also applies
to initiating claims where the injured employee
was on TDY outside of the U.S. or U.S. Territory
at the time of injury or exposure, regardless of
the length of TDY activity. The significant
factor is that the injury occurred outside the
U.S.
7If the employee believes his regular job in the
U.S. or U.S. Territory caused or aggravated the
condition (as opposed to a location outside of
these areas), the claim should be filed in the
district office that has jurisdiction over the
state where the claimant works.
8Claims form completion is basically the same no
matter where the injury occurs, or where the
injured employee lives or works. However, there
are some differences when completing the claims
with regard to the injured employees mailing
address and the place of injury. When entering
the claim electronically all information is the
same until you get to Block 7. Here, you should
use the address where the injured employee is
most likely available to receive correspondence
concerning the claim. If they are remaining
overseas, use their official government address.

10If the injured employee does not have an official
mailing address on post. and uses an address for
their home that is off post When you enter the
data field concerning the State section of the
address, enter FA. By entering FA, you are then
able to use the Control L function to look up the
Country Zip Code. An example is as follows.

12When entering data in Item 9 on the EDI claims
form, enter the data as usual. When you get to
the section that requires a zip code you will be
required to enter the foreign zip code. Foreign
zip codes are 3 Numeric Characters and One Alpha
Character. If you do not know the foreign zip
code you can enter 111A. As shown below, a
list of Foreign Country Zip Codes starting with
Afghanistan and ending with Zimbabwe pops up.
You simply need to find the one you are looking
for, highlight it and click ok.

14After you add a country code, another pop-up will
appear and it will inform you that all foreign
claims must be sent to the OWCP District 9 Office
in Cleveland, Ohio. District 9 will also be
automatically entered in the appropriate block on
the form so you do not have to remember to enter
it later. You will also note that the country of
injury is placed in the Optional Information
Field that is just prior to the zip code entry.

16EDI TELEPHONE NUMBER FIELDS Telephone number
fields in the EDI CA-1 and CA-2 forms will allow
for up to 16 digits (only digits - not alphabetic
or punctuation characters) or allow as few as 9
digits in the telephone number fields.
17Foreign Address Country Codes The address
validation routine will also provide you with a
list of values for Foreign Country codes accepted
by OWCPs EDI process. To obtain the Foreign
Address Country Code listing, simply type FA
(Foreign Address) into the State Abbreviation
field for any of the data fields noted above.
Place your cursor in the corresponding Zip Code
field and use shortcut keys CTRLL to retrieve a
list of values.
18You may search in the list of values for a
particular country code by typing any portion of
the country name AFTER the percent () sign in
the FIND field. Place a percent sign () after
the text that you enter, and then click the FIND
button. To enter the appropriate country code
into the Injury Zip Code data field, simply click
on the selection and then click on the OK button.
(You may also double-click on the selection).
19Show me the Country Codes! NOTE This list
is not all-inclusive, and CPMS ICUC is working
with OWCP to update its foreign country code
listing. In the meantime, particularly with
Germany cases, you must use the Country Code for

20MEDICAL BILL PROCESSING Foreign nationals and
U.S. citizens receiving medical services in a
foreign country are entitled to full medical
benefits, including a choice of qualified
physicians. Particular forms are not necessary
for payment of foreign bills. They also do not
require a provider tax identification number or
procedure codes. Medical bills are submitted to
District 9, Cleveland. The Special Claims Unit
Staff reviews the bills prior to submission to
the National Office for payment. The National
Office facilitates payments of ALL bills.
21MEDICAL BILL PROCESSING (cont.) Although Part 5
of the FECA Manual states that no specific forms
are required, DOL considers that to mean that no
billing forms are required of the doctors like
a HCFA 1500 Form as they do in the USA. DOL does
need a Form OWCP 915 which the CLAIMANT must
complete not the doctor. The 915 is used by
DOL to know the party to pay (claimant or
provider) or where to send the payment.
Claimant must specify on the OWCP 915 Form if
they want payment in dollars, euros, etc.
22 MEDICAL BILL PROCESSING (cont.) Claimant must
attach the medical bill to the OWCP 915 Form and
if at all possible have the medical bill
translated with at least the diagnosis and the
providers name and address. Remind employee that
DOL does NOT pay for translations. Translations
do NOT need to be completed by a
professional. DOL does not use a schedule fee
when processing overseas medical bills. If the
employee was provided medical services by a
Military Facility, provide the Military Facility
with a copy of a CA-16 and the DOL claim number.
Neither the employee, the Agency nor DOL is to be
billed for medical services at a Military
Facility.
23Assistance for all overseas claims can be

522-2786 or DSN 426-2786. If an employees duty
station is in the U.S. and the claim is being
entered after the injured individual has returned
to the U.S., assistance can be obtained by
contacting the Liaison responsible for the OWCP
District where the injured employee works. Note
that after the case is adjudicated the case will
be transferred, if appropriate, to where the
employee resides.
