Why Collaborate and With Who?
  • The TANF program is called MFIP (Minnesota)
    Family Investment Program)
  • Olmsted County has an average of 730 active
    cases per month. We generally process 50 new
    cases each month.
  • The MFIP program is the state welfare program
    for low-income families with children. MFIP
    helps families move to work and focuses on
    helping families move toward self-sufficiency.

Why Collaborate and With Who?
  • In working with our families on the MFIP
    program we found many families had multiple
    barriers to employment. We also found there were
    multiple agencies working with the same families.
  • Our collaboration started as a result of the
    needs of the agency as well as the client.

What Is Family Case Planning?
  • A Family Case Planning Conference is one of a
    number of processes used to bring the familys
    voice, resources and wishes together with their
    service providers to share information and
    develop a comprehensive plan for the family

What is Family Case Planning?
  • A Case Planning Conference is requested by the
    case manager or social worker
  • Is lead by a facilitator, allowing service
    providers to fully participate without leading
    meeting or taking notes
  • Is time-limited, typically 1 to 1 ½ hours in
    length
  • Utilizes an agenda developed by family and
    service providers to move the case forward and
    share most critical information
  • Provides a list of next steps or action steps to
    be accomplished by team members before next
    meeting
  • Generates a written report immediately after the
    meeting, listing names and roles of team members,
    agenda, nest steps and next meeting date so
    everyone has the same information and
    accountability
