• Instructor completes Incident Report and faxes to
  Nursing/Allied Health Office 805-546-3961
• Student completes Students Statement on Incident
  Report and faxes to Nursing/Allied Health Office
  805-546-3961

If health care student wants to see a doctor, but
not an emergency

• Instructor immediately completes Incident Report
  and sends Student to the Health Center or
  approved urgent care facility

TAKE CARE OF THE STUDENT

• Call Public Safety, Ext. 3911
• Public Safety will call 911
• Director is notified immediately
• Human Resources is notified Ext. 3129 at first
  opportunity
