Welcome to the CHIP (Utah Childrens Health
Insurance Program). Many working Utah families
who do not have health insurance may qualify.

3's of CHIP
Applying for CHIP is the first step.

Before you can get medical services, you need to
select a health plan. You can do this at the end
of this slide show or contact a HPR (Health
Program Representative) to help you. (You will
need your case number.)

Once you qualify for CHIP and select a health
plan, your health plan will mail you an
information packet and your new CHIP
medical/dental cards.

CHIP has health plans to choose from. The
benefits are the same no matter which health plan
you choose. If you do not choose a health
plan, Molina will be chosen for you.

6Both health plans are offered in every county.
All you need to do is choose Molina or PEHP. The
health plan you choose will add you to the CHIP
plan they offer in your county.
7Choosing a Health Plan
After you are approved for CHIP, you will receive
a letter asking you to pick a health plan. You
must choose either PEHP or Molina as soon as you
are approved so that you can take full advantage
of the benefits and services that CHIP offers.

of provider choices - Process your medical claims
- Pre-authorize procedures when needed

9HPRs will...
- Offer free classes about how CHIP works
- Help you choose a health plan
- Answer questions about your health plan
- Provide you with helpful materials about CHIP
- Give you information about changing your
providers or health plan - HPRs are located at many of the
- Department of Workforce Services (DWS) offices.
HPRs cannot answer questions about your CHIP
eligibility.
10Changing a health plan?
You must stay with your selected health plan
through the end of June each year. CHIP allows
you to change your health plan each year May
through mid June. The change would start on July
1st. You will receive a letter each year to
remind you of the plan change period.
11If you move to a different county within Utah,
you must call your HPR because your current
health plan may not be available in your new
area.
12Premiums
Depending on your income, you may need to pay a
premium. CHIP will bill you when quarterly
premiums are due. See your member guide for more
information about premiums and instructions on
how to pay your premium. If you do not pay your
premium on time, your case may be closed and you
will have to wait until the next CHIP open
enrollment to reapply.
13Co-Pays
Co-Pays -Most CHIP families will have co-pays for
medical and dental services. Your eligibility
worker will assign you to CHIP Plan A,B, or C
based on your income. The co-pay plan will be
listed on your CHIP ID card.

14Deductibles
Deductibles Plan A and B do not have a
deductible. Plan C requires that a deductible be
paid before benefits begin. The deductible
applies to inpatient, outpatient hospital and
major diagnostic services.

Your health plan will send you a booklet about
your benefits.

Helpful Hint Get your case number from your
eligibility worker. The application confirmation
number will NOT be your case number.

selects Molina for their health plan will be
covered by Molina Healthy Kids Plus. If they
select PEHP, they will be covered by PEHP
Exclusive. Remember that CHIP benefits are the
same no matter which plan you choose.
