5Assessment Reports
- Gives you an assessment of the clients current
immunization history within 4 different age
categories. - lt12 months
- 12-23 months
- 24-35 months
- 36-72 months
- Numbers only report- no client specific
information
6What information do you need to run the report?
Evaluation Date used to set the age range for
the clients in the report.Client Population
options are available based on what type of
provider is requesting the assessment.

8How Do I Run an Assessment Report
Choose your client population
Select your evaluation date
Remember!!Only click Generate once
Click Generate when done
Choose assessment report
9Status Report!!!
Stage 1
If Status is Complete click on report name
10Go to File/ Save As and save the report.
Remember to pick a name youll remember
11How Will This Assessment Report Help Me?
- Numbers only report- helps you see your
immunization rates right away - Keep track of your clients immunization
status. Are they up to date? Do they need to come
back in? - Show the number of clients that could have
been brought up to date with additional
immunizations. - Assessment of missed opportunities.
12Request Reminder
- Serves to generate mailing reports, address
labels, or client listing for clients in their
organization. - Unlike the Assessment report, Request Reminder
will show client specific information.
13Things to Remember
- Update Responsible Person information
- No more than 4 responsible people
- Check the Notices box
- Delete unnecessary notices

- Going to help you determine which clients are
not up to date with their immunizations. - Provide a fast way to run a report with all
the contact information for those clients that
need to come back in. - Improve the immunization rates because you
will be able to keep your clients up to date.

(94.44) were not up to date and 10 of them
(58.8) were considered to have had a missed
opportunity.
So, how do I find these kids?

Enter the birth date range from the assessment
report
Click Generate when done
Finish with Reminder Recall

- What do I do with the Omitted Clients?
- Print the omitted client list
- Go back to Manage Client
- Add the responsible person information
32Things to Remember
Every time you change a listing factor the
Reminder Recall must be run again
Before you run the Reminder Recall again, lets
look at the next query, which is the client query
listing, to see if you can update more
information in the clients record

clients, you will come back to the status page

34What can you do with the Client Query Listing?
- Use it to look up chart or EMR copies of
immunization records to ensure that all
historical doses are entered - If you want to fix those records- you can go back
through Manage client and add those historical
doses

Clients
The client query listing is the best way to find
those clients for a certain age group in the
assessment report.
- From here you have two option
- Take the omitted list and the client query list
and clean up those records as much as possible - Take notice of those that are omitted, and the
clients in the client query listing and go back
later and fix them
How do I fix the Omitted Clients???

Go back to Manage Client
Take the first client on your omitted clients
list and plug them into the search screen
37Last Name, First Name, Relation, and either a
phone or complete address is required
