Aim

• Richland Community Health Care will redesign
  its current health care system for patients with
  Diabetes to reduce mortality associated with this
  chronic disease. We will do this with the
  implementation of the Care Model and will use the
  national measures provided to determine
  effectiveness.

Population of Focus

• 150 patients in care of Dr. Gold.
• These will be 150 patients diagnosed with
  diabetes.
• Pts selected to be a part of this program to be
  given a chance to fully improve their lifestyle
  despite discrepancies brought on because of their
  diagnosis.

• Populated by Dr. Golds patients
• IT staff installed PECS software. After pts were
  selected they were entered into the PECS system
  by the data specialist. (Demographics only)
• 96 pts currently in registry
