Title: Bedtime Basics for Babies

1
Bedtime Basics for Babies
  • An Initiative of the National Crib Campaign

2
First Candle Background
  • Received 11 million grant from Bill Melinda
  • Gates Foundation in 2007
  • Washington State
  • Indiana State
  • Washington, DC

3
The Gates Initiative Need
  • 4,500 babies die yearly in US suddenly/
    unexpectedly
  • At least 50 of these deaths may be caused by
unsafe sleep environments and, therefore, may be
    preventable
  • Risk of suffocation is approximately 40x higher
for infants sleeping in adult beds than those in
    cribs
  • Percentage of infants bed sharing all night has
    doubled from 5.5 to 12.8 since 1993
  • 44 of infants spend some time sleeping in adult
    bed

4
Goal/Objectives
  • Reduce SIDS and sleep-related infant deaths by at
    least 50 over a seven-year period
  • Distribute Bedtime Basics kits (crib, sheet,
    wearable blanket pacifier) to high-risk
    families
  • Deliver safe sleep messages to new/expectant
parents, healthcare providers and the general
    public
  • Conduct a comprehensive evaluation of the data
  • Publish and widely-share the results

5
The Gates Initiative Scope
  • Duration 7 years
  • Target Audiences
  • High-risk families with low SES status and 1
risk factor for SIDS and sleep-related death
  • Health and social service professionals
  • New and expectant parents
  • The general public
  • Staffing Managing Director, Administrative
    Assistant, 3 Project (site) Directors

6
The Gates Initiative Implementation
  • Legal, financial and insurance audits complete
  • Partners in place (Indiana Univ., Riley
    Childrens, Seattle Childrens, Urban Indian
    Health, DC DOH)
  • IRB submission and approvals Nearly complete
  • Survey instruments nearly complete

7
The Gates Initiative Implementation
  • Educational materials complete (focus groups,
translation, worked with National Center for
    Cultural Competence)
  • Coalitions formed and in operation in IN and DC
    under development in WA state
  • Nursing trainings (250) complete in IN being
    scheduled in DC and WA

8
The Gates Initiative Health Messages
  • Follow AAP guidelines, complement existing First
    Candle campaigns messages
  • Committed to being culturally sensitive and
competent (working with National Center for
    Cultural Competence)
  • Will address diverse language needs
  • Proactively encourages breastfeeding (working
with breastfeeding coordinator of the Indiana
    Perinatal Network)

9
Evaluation
  • Investigators
  • Dr. Rachel Moon, Childrens National
  • Medical Center and George Washington University
  • Dr. Fern Hauck, University of Virginia
  • Dr. James Kemp, Washington University
  • Track both process and outcome data
  • Use both qualitative quantitative data
    collection tools

10
Hypotheses
  • The provision of cribs will result in a
significant reduction of sudden infant deaths
  • Wide-spread distribution of safe sleep messages
will increase knowledge of safe sleep practices
  • There is not one best practice, but many and
effective models are best determined and
    implemented by each site

11
Data to be Collected
  • Aggregate demographic and infant mortality data
    from each site
  • Surveys of participants (upon enrollment,
    post-education)
  • Pre- and post-training surveys for nurses
  • Surveys to collect data from distributing
    agencies
  • Housed in a web-based database

12
Collection of Aggregate Data
  • Outcome Measures
  • Infant mortality rates
  • SIDS and sleep-related death rates
  • Compare data in years 3, 5, and 7 with baseline
    data

13
Participant Surveys
  • Survey before/at time of crib distribution
  • Knowledge about safe infant sleep recommendations
  • Practices/planned practices re safe infant sleep
  • Follow up survey 1-3 months after crib given
  • Knowledge, practices
  • Evaluation of the process suggestions for
    improvement
  • Families can receive crib without participating
    in surveys
  • Written informed consent will be obtained

14
Nurse Surveys
  • Nurses will complete surveys before and after
    completing training module
  • Compare knowledge and intended practices

15
Distributing Agencies Surveys
  • Process evaluation
  • What works?
  • What doesnt work?
  • How can we make it work better?
  • Assist with inventory, allocation of materials
  • Numbers of cribs, other materials given out
  • If distributing agency is providing more cribs
    than others, can adjust allocations

16
Web-based Database
  • All of the data will be entered onto a web-based
    database
  • Adequate disk space
  • Excellent security of data
