5New Medical Errors Report
- A new study (HealthDayNews) has revealed that
medical errors kill 195,000 Americans each year
twice as many that were reported by the Institute
of Medicine in its 1999 report which indicated
between 44,000-98,000 Americans die each year
from medical errors. - The leading cause of death due to medical errors
is caused by patient misidentification and
specimen or medication misidentification. - It is estimated that medical errors result in
17-29 billion dollars in national costs. These
include lost income, lost household production,
disability, and health care expenses.

personal and medical information from being
improperly utilized

penalties of 100 per failure to comply.

10JCAHO guidelines require positively identifying
patient before administering care to eliminate

- Identify the individual as the person for whom
the treatment/service is intended - Match the treatment/service to that individual
- Two (2) patient identifiers (individuals name,
assigned identification number, telephone number,
or photograph) must be directly associated with
the individual and the same two identifiers must
be directly associated with the medication, blood
product or specimen tube. (i.e. an attached

11FDA Mandate aims to confirm patients are
administered the proper medications, in the
correct dosage
- The FDA issued a new rule to require certain
human drugs and biological product labels to have
bar codes. The bar codes must contain the
National Drug Code (NDC) number in a linear bar
code. (Minimum requirement) The lot number and
product expiration may be added at companies
discretion. - Blood and blood component labels must include FDA
approved symbols identifying the collecting
facility, the lot number relating to the donor,
the product code and donors blood group and type.

Pathologists on February 2002 reviewed wristband

there were 45,197 errors found.
