New Medical Errors Report
  • A new study (HealthDayNews) has revealed that
medical errors kill 195,000 Americans each year
twice as many that were reported by the Institute
of Medicine in its 1999 report which indicated
between 44,000-98,000 Americans die each year
    from medical errors.
  • The leading cause of death due to medical errors
is caused by patient misidentification and
    specimen or medication misidentification.
  • It is estimated that medical errors result in
17-29 billion dollars in national costs. These
include lost income, lost household production,
    disability, and health care expenses.

6
Bar code Technology helps prevent
misidentification, which can save lives
  • Results from the VA Medical Center and other
medical centers prove that bar code products used
in combination with medical management systems
can reduce the frequency of medication errors and
    enhance patient care.

7
VA Research Conclusion
  • 86.2 reduction in medication errors (including a
75 improvement in errors caused by the wrong
medication being administered to the patient).
  • 93 improvement in errors caused by the incorrect
administration of dosage to the patient.
  • 70 improvement in errors caused when medication
scheduled for administration were not given.

8
Government Regulations/Recommendations
  • HIPAA Health Insurance Portability and
    Accountability Act of 1996
  • JCAHO Joint Commission on Accreditation of
    Healthcare Organizations
  • FDA Food and Drug Administration ruling on Bar
    Coding for the Pharmaceutical Manufacturers
  • The Institute for Safe Medication Practices in
    Huntington Valley, PA
  • Recommends in the ISMP Patient Safety brochure
    Do not let anyone give
  • You medicine with out checking your patient
    identification or bar code
  • Bracelet.

9
HIPAA Requirements are designed to prevent
personal and medical information from being
improperly utilized
  • Protected Health Information (PHI)
  • Individually identifiable health information
  • Individually identifiable health information
    includes common identifiers name, address,
    birth date, social security number.
  • Civil Money Penalties- HHS may impose civil money
penalties of 100 per failure to comply.
  • Criminal Penalties a person who knowingly
    obtains or discloses individually identifiable
health information in violation of HIPAA faces a
fine of 50,000 and up to 1 year imprisonment.
  • Criminal Penalties increase to 100,000 and up to
5 years imprisonment if the wrongful conduct
involves false pretenses and to 250,000 and up
to 10 years imprisonment if the wrongful conduct
involves the intent to sell, transfer, or use
    identifiable health information for commercial
advantage, personal gain or malicious harm.

10
JCAHO guidelines require positively identifying
patient before administering care to eliminate
mis-medicating
  • Identify the individual as the person for whom
    the treatment/service is intended
  • Match the treatment/service to that individual
  • Two (2) patient identifiers (individuals name,
    assigned identification number, telephone number,
or photograph) must be directly associated with
the individual and the same two identifiers must
be directly associated with the medication, blood
product or specimen tube. (i.e. an attached
    label).
  • __________________________________________________
    __
  • Bar Coding that includes 2 or more person
specific identifiers will comply with these
    requirements.

11
FDA Mandate aims to confirm patients are
administered the proper medications, in the
correct dosage
  • The FDA issued a new rule to require certain
human drugs and biological product labels to have
bar codes. The bar codes must contain the
National Drug Code (NDC) number in a linear bar
code. (Minimum requirement) The lot number and
product expiration may be added at companies
    discretion.
  • Blood and blood component labels must include FDA
    approved symbols identifying the collecting
facility, the lot number relating to the donor,
the product code and donors blood group and type.

12
Wristband Errors
  • A study published by the College of Medical
Pathologists on February 2002 reviewed wristband
errors over a 2 year period.
  • Of the 1,757,730 wristbands that were examined,
    there were 45,197 errors found.

13
The Results of the Wristband Study
14
Positive Patient ID Starts Here
Personal ID Patient identification wristbands
have set a new standard. Durability, Security,
and Overall Patient Safety

16
Patient ID Wristbands High Resolution Surface
  • Personal ID wristbands produce extremely sharp,
high contrast, high resolution bar codes (linear,
2-D and RSS symbologies) that deliver 1st time
    scans with no delays
  • Patient images are also printed high resolution
up to 609 dpi for crystal clear images

17
Benefits of a Photograph
  • Consider the benefits of photos on driver
licenses and passports. Without a photo, positive
    identification cannot be assured

18
Benefits of a Photograph
  • The patients image on the Personal ID wristband
acts as an additional patient identifier, helping
    to avoid errors due to
  • Multiple patients have the same or similar names
  • Patients unable to reliably confirm their
    identity when asked

Patient ID - the Software Solution
Patient ID is an inexpensive, user friendly
application for producing patient
identification wristbands or labels.
The Patient ID software is easy to install,
using a client based application, does not
require an additional server to be installed or
supported.
The Patient ID software can capture patient
admission data through a workstation via a
RS-232 port, TCP/IP or from a data file
produced by the hospital admission system.

FAQ
  • What sizes are available for the
    wristbands?
  • Adult, adult/pediatric and infant
  • What colors are available for the
    wristbands?
  • Adult and Adult/Pediatric - white, blue,
yellow, green, red and orange. Infant sizes
    white, pink and blue.
  • Does the curvature of the wrist interfere
with the scanning of the bar code?
  • It can. If too many characters contained
within the bar code causes the bar code to be
too long, and curves around the wrist.
  • Why are adhesive based wristbands a problem?
  • When adhesive is present in
    wristbands, it can attract bacteria,
microbes, body hair, skin cells, and other
    contaminants. In addition,
  • many adhesives are easily defeated and the
    wristband can be removed.
  • Can RMS Omega provide integration
    services?
  • RMS Omega can provide complete
integration services that enable you to print the
Personal ID wristband directly from your
existing HIS, LIS, or admission system.
