This spinner is used to unscrew the drill-pipe
from the string.When done the pipe is lifted,
the spinner is released and the drill-pipe is
pulled away by hand to be set down on the floor.

This is what happenedThe floor-man placed his
hand on the joint BEFORE the spinner was
released. Spinner was disengaged and dropped a
few inches (normal).His finger was crushed and
badly cut between the spinner and the drill pipe.
He will be off work for several weeks.

What caused this LTI ??- Hand placed where it
should not have been!- Hazard/danger not seen.-
Lack of concentration/attention.- Poor
communication between driller and floor-man-
