Health Risk Assessment Events

• Blood Analysis
• Total Cholesterol
• HDL LDL
• Triglycerides
• Glucose
• PSA (males 45)

• Completed Survey
• Biometrics including
• Blood Pressure
• Waist/Hip Ratio
• Body Mass Index (Height Weight)

NO drug testing will be performed.

Individual Reports

Compliance

• To be considered a compliant participant in the
  program employees must follow the guidelines
  below
• Participate in the Health Risk Assessment event
  sponsored annually by Employer (mandatory)
• Each participant must complete steps outlined on
  individual Healthy Life Plan within the time
  frame allocated
• Individuals with a BMI (Body Mass Index) of 30 or
  higher AND/OR a Waist-to-Hip Ratio of 1.09 or
  higher must participate in a Weight Management
  Program and document involvement on monthly basis
  with the on-site NP
• The Program is based on Action NOT Outcome

Role of the Nurse Practitioner (for employees
within the Raleigh Area)

• Nurse Practitioner Services
• Diagnosis and treatment of minor medical
  conditions
• Disease management
• Order diagnostic testing services
• Blood draw and specimen analysis
• Prescribe medications and write prescriptions
• Nutritional and exercise advice
• NO drug screening NO workers comp filing

On-site Clinics (for employees within the
Raleigh Area)

• Clinic locations
• 5th floor of the DPI Building, Office 595
• The Division of Workforce Development, Chappanoke
  Drive, Office 174
• To use Clinic Raleigh-based employee, come
  through HRA, and remain compliant in the program
• Employees will not have to use any leave for
  scheduled appointments at the clinic.

Personal Health Information

• Information collected during the Health
  Assessment event, by the Nurse Practitioner, and
  from health plan claims data will NOT be shared
  with anyone without your written consent.
• No representative of your Employer will ever have
  access to this information.
• This commitment is consistent with the Health
  Insurance Portability and Accountability Act
  (HIPAA) passed by the U.S. Congress.

UNC Pilot Study

• This wellness program is a 2 year program
• Certain components of the program are being
  measured by UNC through de-identified
  information
• NO personal information will be shared with UNC
  (NO names, NO Dates of Birth, NO Social Security
  numbers, etc.)
