• Location based on the health needs of the
  population
• Life expectancy in Darlington is below the
  national average
• There are significant inequalities in life
  expectancy within Darlington
• Death rated from coronary heart disease and
  cancer are above the national average
• Levels of smoking are higher than the national
  average

• Teenage pregnancy rates significantly higher than
  the national average
• The incidence of sexually transmitted infections
  are higher than the national average specifically
  amongst young people
• Significant unmet need barriers to accessing
  primary care services for people with learning
  disability

• Unmet need in terms of access for people with
  severe mental health problems
• Proposed location is in Dr Piper House to
  co-locate with the Walk in Centre
• Consultation on the location taking place and due
  to end on the 4 August
• The provider will be expected to demonstrate how
  the Health Centre will integrate with the
  services already provided by the Walk in Centre
• The provider will be expected to engage in
  practice based commissioning at an individual
  level or in commissioning groups
