Pre-hire Paperwork
2Hello and welcome to the AmeriCorps Watershed
Stewards Project (WSP)! This slide show is
intended to walk you through the pre-hire
paperwork and screening processes required to
become a WSP member in January. As a reminder,
all pre-hire paperwork is due to the WSP office
by Monday, December 15th. If you have any
questions regarding this paperwork, please dont
hesitate to contact the WSP staff at (707)
725-8601 or the email addresses below. Also,
there will be two mandatory orientation
conference calls in which all members are
expected to participate. We recommend
participating in each call live, although you can
call in and listen to the recorded version at a
later date if that is not possible. A secret
password will be given out at the end of each
call which all members will be required to email
to Lindsay to verify you have listened to

Call B Pre-Service Orientation Monday, January
5th, 1000am PST Please call 866-643-0665 and
use Participant Passcode 9220060. Best of

3The Authorization for Release of Driver Record
Information Form discloses to the California
Conservation Corps (CCC) if a member obtains any
driving violations during the WSP service year.
In other words, if you get a DUI, please notify
WSP know before the DMV does.
4The Request For Live Scan Service Form is
necessary to obtain the Live Scan fingerprint
background screening required by the CCC and
AmeriCorps (AMC). The Live Scan screening
ensures that we are employing AmeriCorps members
without a criminal background. WSP members will
be working with the public and children, so
safety is a priority!
5Live Scan Rolling Fee Form is available for
members that would like to get a Live Scan
screening completed (with the fee waived ) at the
Eureka Police Department. However, this form
needs to be signed by Carrie or Lindsay, so
please contact Kristy, WSP Member Coordinator, if
you are interested in utilizing this option.
6AmeriCorps Trainee Agreement needs to be signed
by the trainee (soon to be WSP member - YOU)
indicating their agreement of the conditional
acceptance of employment as a CCC AmeriCorps
member. The contract will be upheld as long as
all of the pre-employment hiring conditions are
met.
7Signing the Addendum To The CCC Trainee Agreement
indicates that the trainee (soon to be WSP member
- YOU) is declaring, under penalty of perjury,
that the potential member is not on probation or
parole, nor been convicted of crimes.
Furthermore, signing this document means that the
potential member understands that the results of
the background screening sometimes take awhile,
and if the results do not meet the pre-employment
hiring conditions, the members employment will
be immediately terminated.
8The Job Analysis Form works in conjunction with
the Health Questionnaire and helps explain some
of the physical job activities that may be
required while working at the placement sites.
This form is referred to by health care
professionals to assist in evaluating a members
ability to perform the job functions in the
pre-hire physical.
9The Health Questionnaire is a confidential
document that assists the health care
professional in evaluating the member during the
pre-hire health screen. The completed
questionnaire needs to be forwarded to WSP in the
SASE envelope provided.
10The Mailing Checklist is a way to help make sure
you are mailing everything you need to by the
deadline December 15, 2008!
