Cedar Lake Residences
  • Cedar Lake Residences community based
    services began in 1989 with the opening of One
    Hundred North Fourth LaGrange group home. By
    late 1993 we expanded community services into
    supported apartment options and Cedar Lake
    Residences was incorporated. Since that time CLR
    has grown steadily by developing a variety of
    community services. Currently CLRS 100 staff
    members have the capacity to serve over 120
    individuals with mild to moderate intellectual
    disabilities. The age range of CLR clients is
    from 21 to 67 years old, the average age being
    44. Funding for services comes from a variety of
    sources that include Kentuckys Supports for
    community living waiver, an individuals own work
    earnings, HUD rental subsidies, government
    benefits such as Social Security or SSDI, private
    pay from family members or trust funds, and Cedar
    Lake Foundation need based financial assistance.

  • 1989-Cedar Lakes first community residence,
    Group Home is dedicated.
  • 1993- Cedar Lake Residences begins distinct
    operations with independent incorporation.
    Independent supported living program begins with
    three clients in rental property in LaGrange.
  • 1994- Corporate reorganization creates Parent
    Cedar Lake, Inc. and new subsidiaries of Cedar
    Lake Residences, Inc. and Cedar Lake Abigail, Inc
    to join restructured Cedar Lake Lodge and Cedar
    Lake Foundation.
  • 1996-Cedar Lake Residences begins in-home support
    services.
  • 1997-CLR apartments at 2711 Riedling become first
    integrated housing
  • 2003-Cedar Lake Residences opens its first
    staffed residence at 1009 Richwood Way in
    LaGrange for three women.
  • 2005-CLR opened 2 more staffed residences,
    Falmouth, and Richwood II.
  • In January 2006 Cedar Lake was awarded a 1.1
    million from HUD Housing and Urban Development to
    purchase and open 3 new staffed residences in
    Metro Louisville.
  • In October of 2007 Cedar Lake was awarded their
    9th HUD grant of 1.2 million dollars to build 3
    new staffed residences in Oldham Co.

Group Home
  • This option is a semi-independent, twenty four
    hour a day staffed home for six residents. The
    residents work or volunteer. This community
    residence opened in 1989. Its gracious style
    blends well into the LaGrange community.
  • Typically for individuals with mild or moderate
    intellectual disabilities. Residents receive
    training and support in such areas as daily
    living skills, transportation, leisure and
    recreational activities, as well as benefits
    coordination.

The Group Home

24 Hour Staffed Apartments
  • The staffed apartments option was created to
    fill a gap for individuals with moderate
    intellectual disabilities who still want or need
    staff supervision on a 24 hour a day basis, but
    prefer the advantages and semi-independence of
    living with a roommate in their own apartment.
    These apartments are located in the Clifton
    neighborhood of Louisville. Those who experience
    success in this living environment often become
    better equipped to live independently in their
    own apartment. Residents are typically involved
    in work or community activities on a daily basis.

Staffed Residence
  • This option provides 24-hour care for three
    individuals in a family type atmosphere.
  • Cedar Lake currently operates 12 staffed
    residences.

Cedar Lake-Jefferson Manor
  • This option consists of two three bedroom patio
    homes which are staffed (12) hours a day. It is
    designed to provide a level of support between
    the group home and independent living. The six
    residents work in the community at least 20 hours
    a week.

In Home Supports
  • This option is designed to provide supported
    living services to persons who reside in their
    own or family homes.
  • Service plans are developed on an individualized
    basis to meet client and family needs, and to
    encourage independence.

CLEC the day program
  • Currently has 37 participants and 6 full-time
    CLEC staff
  • a director, a registered nurse, 2 program
    coordinators and 2 program assistants. Two of
    CLRs 1st shift staff come to support the program
    on a daily basis as well.
  • There is program curriculum in place specific
    to each clients personal goals. The program
    curriculum is comprises of a vocational,
    devotional-spiritual, pet therapy, nature
    education, creative arts, adult living skills,
    health and wellness, reading group, and community
    experiences to name a few.

Supported Independent Living
  • Supported Independent Living Apartments in
    LaGrange and Louisville, which are owned by CLR.
    ( several with HUD funding)
  • CLR has 8 apartment locations
  • 41 clients who receive support for independent
    living.
  • This program provides supported independent
    living options for persons with mild or moderate
    levels of intellectual disabilities who desire to
    live independently in the community. Cedar lake
    staff provides a minimum of 10-20 hours of
    support services a month. Individuals work or
    volunteer in the community at least 20 hours per
    week. Clients typically receive supports in areas
    such as nutrition planning, grocery shopping,
    financial management, social skills developement,
    homemaking, safety skills, and benefits
    coordination.

Future Growth

  • Cedar Lake Residences has continued to expand our
    service system to add both capacity and living
    arrangement options. In October 2007 CLR received
    our ninth HUD grant, for 1.2 million to develop
    three additional staffed residences providing
    residential and support services to nine
    individuals. This was the largest HUD 811 grant
    awarded in Kentucky last year. Plans are underway
    to have the homes developed and operational by
    mid 2009. As a transitional step toward opening
    these new HUD homes, CLR recently rented two
    homes in Henry County to serve six additional
    individuals.
