What happened RWC-EMPLOYEE STRUCK BY KELLY HOSE
STANDPIPE TREE- While removing standpipe tree,
(top and bottom unions loose) EE was struck in
the chest and head, when the tree released.
Tension was held on tree via air tugger. IP was
transported to a medical clinic for evaluation.
All x-rays were negative, with 6 sutures required
for a cut to the IPs forehead.

• No JSA for the specific task.
• No procedures for specific task.
• Establish line of fire zones/paths and
  maintain them.
• Train Crews on the proper tree removal steps.
  This task could have been completed without
  tension on the tree.
• Production focus although rig was released,
  there is evidence of the crew rushing the job.
