Be sure

• That you are connected directly to your FrontPage
  site on the server (not just on the local
  machine)
• Be sure that the Page view is selected
• Go to FilegtNewgtPage and choose Form Page Wizard
• There, youre half done!

The Wizard
With this, you mostly just click Next to get
you through the process

Start Adding Items
When you click Add youll be given a list of
stuff to add IMPORTANT! Anything you add using
the Wizard can be changed later.
Click Add

Start Adding Items
Select your first category of information
Lets assume that you are getting information on
a person. Lets bet that Contact Information
will capture some of the info you want

Lets Select Contact Information as the First
Category of Information that we want
The type of input called contact information
is selected
We can edit the question that the visitor will
get here or we can do it later.

Customizing the First Box
You can uncheck and check boxes as you need
The wizard is going to group Input into
variables and you Can control the name of
variables Here if you want

One Possible First Box
Ive said I want first and last name separately
and Ive done away with some of the things like
title, organization and fax Ive also entered a
new base name that Im calling BaseInfo
Now Ill click Next

Add More?
This looks familiar. Its the same as the first
dialogue box we saw except that it contains the
info we just asked for in the last dialogue box

If We Need More Info, We Just Click Add

Add More Questions Via the Wizard
How would you collect more information
automatically? You guessed it! Click on
another category. Notice that you can pull the
right bar down to see more possibilities.

Wizard
We can change this statement later so well not
fuss about it now

uncheck Name

Name is unchecked. Well ask for Age, tell it
we want Age entered as years old and also
ask for Sex (gender) and ID Number

You can click the Add button as many times as
you need. Well click it once more and add a
paragraph where well ask the respondent to tell
us something

Were told that were putting In a scrolling text
box with no Length restrictions
We must give a name to the place that will hold

Because we didnt save as, FrontPage saved the
form as new_page_1.htm
formrslt.htm is the file FrontPage created to
hold results

Click on preview in browser button to test

John Doe Fills Out Form
When John hits the Submit Form Button, the
info Will go to the formrslt.htm file

John immediately sees a confirmation that
his submission has been sent
