Step 1 Determine whether you are currently
completing a Flex Program evaluation

• Flex Program ? Critical Access Hospitals
• Evaluating the Flex Program ? evaluating Critical
  Access Hospitals
• Evaluating the Flex Program evaluating the
  outcomes and impact of the Flex Program in each
  state

Step 2 Create or Revise Your State Flex
Evaluation Plan

• Get a copy of the evaluation toolset from TASC
• Available in December 2002
• Will be sent to all Flex Program Coordinators
• Review your Flex Program implementation plan (as
  stated in your 2003 federal Flex Program grant
  application)
• Identify your state Flex Program implementation
  areas to be evaluated
• Revise your evaluation plans as needed
• Evaluation plans are to be submitted to Forest
  Calico at the FORHP no later than Dec. 31, 2002
